IDH1 (isocitrate dehydrogenase 1) is a metabolic enzyme that, when mutated, produces the oncometabolite 2-hydroxyglutarate and serves as a key biomarker in brain tumor classification. IDH1 mutations, particularly the R132H variant, are found in the majority of lower-grade gliomas and secondary glioblastomas but are rare in primary glioblastomas. The mutation status fundamentally alters tumor biology and patient prognosis, making immunohistochemical detection of mutant IDH1 essential for modern neuropathological diagnosis according to WHO classification.
