Aortocaval Compression

Compression of major blood vessels by the gravid uterus when the pregnant patient lies supine, causing hypotension and reduced cardiac output.

Full Definition

Aortocaval compression, also known as supine hypotensive syndrome, occurs when the enlarged pregnant uterus compresses the inferior vena cava and aorta against the spine in the supine position. This compression significantly reduces venous return to the heart, leading to decreased cardiac output and maternal hypotension. The condition can also reduce uteroplacental blood flow, potentially causing fetal distress. In obstetric anesthesia, preventing aortocaval compression is crucial during regional anesthesia procedures and cesarean deliveries. Standard practice includes left uterine displacement or positioning the patient with left lateral tilt to alleviate vascular compression.
