Aortocaval Syndrome

Maternal hypotension and reduced cardiac output caused by compression of the inferior vena cava and aorta by the gravid uterus in the supine position.

Full Definition

Aortocaval syndrome, also known as supine hypotensive syndrome, occurs when the enlarged pregnant uterus compresses the inferior vena cava and potentially the aorta when the mother lies supine, typically after 20 weeks gestation. This compression reduces venous return to the heart, decreasing cardiac output and maternal blood pressure, which can compromise uteroplacental perfusion and fetal oxygenation. In obstetric anesthesia, prevention involves positioning the patient with left uterine displacement or in the left lateral tilt position during procedures. Recognition and immediate correction of this syndrome is crucial during neuraxial anesthesia administration and throughout labor and delivery.
