Lenaerts syndrome, also known as bilateral diffuse uveal melanocytic proliferation (BDUMP), is a rare paraneoplastic syndrome characterized by bilateral thickening of the uveal tract due to melanocytic proliferation. This condition is typically associated with underlying systemic malignancies, most commonly gynecologic or pulmonary cancers. Patients present with bilateral vision loss, often with characteristic fundoscopic findings including multiple round, red patches at the level of the retinal pigment epithelium, diffuse choroidal thickening, and exudative retinal detachments. The syndrome may represent an autoimmune response to tumor antigens that cross-react with ocular melanocytes. Early recognition is crucial as it may herald the presence of occult systemic malignancy.
