An orbital implant is a biocompatible sphere placed within Tenon's capsule following enucleation to replace the volume of the removed globe and provide a foundation for prosthetic eye movement. Common materials include porous polyethylene (Medpor), hydroxyapatite, and silicone, with porous implants allowing fibrovascular ingrowth for enhanced integration. The implant is typically 18-22mm in diameter and may be wrapped in donor sclera, fascia lata, or synthetic materials. Proper sizing prevents enophthalmos and superior sulcus deformity, while motility pegs can be attached to improve prosthetic movement. Complications include exposure, infection, and implant migration.
