Radiation vasculopathy represents a spectrum of delayed vascular complications that can affect both retinal and choroidal circulation following radiation therapy for intraocular malignancies. This condition typically develops months to years after treatment and is characterized by endothelial cell damage, capillary closure, and microangiopathy similar to diabetic retinopathy. Manifestations may include cotton-wool spots, hard exudates, retinal hemorrhages, microaneurysms, and in severe cases, neovascularization and vitreous hemorrhage. The severity and onset are related to the total radiation dose, fractionation schedule, and individual patient factors. Early detection through regular ophthalmoscopic examination and fluorescein angiography is essential for timely intervention with anti-VEGF therapy or laser photocoagulation.
