Desmoplastic ameloblastoma is a distinct variant recognised by the WHO, notable for its dense fibrous (desmoplastic) stroma that compresses and distorts the odontogenic epithelial islands. It most commonly occurs in the anterior maxilla or mandible, a distribution different from conventional ameloblastoma. Radiographically it often presents as a mixed radiolucent-radiopaque lesion, which can cause diagnostic confusion with fibro-osseous lesions. Editors must ensure the qualifying adjective 'desmoplastic' is retained when transcribing histopathology reports, as excision of this modifier changes the diagnostic entity. The WHO Classification of Head and Neck Tumours recognises it as a separate subtype.
