Epithelial dysplasia in oral pathology refers to a spectrum of cytological and architectural abnormalities in the oral mucosal epithelium that indicate a pre-malignant state. It is assessed histologically and graded as mild, moderate, or severe (or alternatively as low-grade and high-grade in some classification schemes). Features include loss of polarity, increased mitotic figures, nuclear hyperchromatism, and drop-shaped rete ridges. Editors must understand that dysplasia is strictly a histological term and should not be applied clinically; its presence does not confirm carcinoma but signals malignant potential. Grading terminology should be preserved exactly as the pathologist has recorded it.
