Keratocysts, also known as odontogenic keratocysts (OKC), are developmental cysts arising from dental lamina remnants characterized by a thin, friable keratinized epithelial lining and aggressive biological behavior. These cysts demonstrate a high recurrence rate if incompletely excised due to satellite cysts and epithelial islands left behind. They commonly occur in the posterior mandible and may be associated with impacted third molars. Multiple keratocysts may indicate nevoid basal cell carcinoma syndrome (Gorlin syndrome). Treatment typically involves enucleation followed by adjunctive therapy such as cryotherapy or chemical cauterization to reduce recurrence risk.
Usage
Usage note: WHO classification now prefers 'keratocystic odontogenic tumor' but 'keratocyst' remains widely used clinically.
