Cortical windowing is a specialized surgical approach used in orthopedic oncology to gain access to intramedullary bone lesions while minimizing structural compromise. The technique involves creating a precisely sized rectangular opening in the cortical bone, typically using oscillating saws or high-speed burrs. This approach is particularly valuable for intralesional procedures, biopsy of deep lesions, or curettage of benign aggressive tumors. The cortical window can often be replaced as a bone graft or left to heal secondarily, depending on the size and location of the defect.
