A scoring system used to assess the risk of pathological fracture in patients with bone metastases, guiding prophylactic intervention decisions.

The Mirels score is a validated clinical tool developed by Dr. Hilton Mirels to predict the risk of pathological fracture in patients with metastatic bone lesions. The scoring system evaluates four factors: anatomical site (upper limb, lower limb, or peritrochanteric region), pain level, lesion appearance (blastic, mixed, or lytic), and lesion size relative to bone diameter. Each factor is scored from 1-3 points, with total scores ranging from 4-12. Scores of 9 or higher indicate high fracture risk and typically warrant prophylactic surgical intervention, while scores of 7 or lower suggest lower risk that may be managed conservatively.
