Cancellous impaction refers to the compressive failure of trabecular bone architecture within the metaphyseal or epiphyseal regions of long bones. This injury pattern occurs when traumatic forces exceed the structural capacity of the spongy bone, causing collapse and compression of the trabecular network. Cancellous impaction is commonly seen in elderly patients with osteoporotic bone and in high-energy trauma affecting metaphyseal regions. The impacted bone may require elevation, bone grafting, or specialized fixation techniques to restore structural integrity and prevent progressive collapse. Recognition of cancellous impaction is important for appropriate treatment planning and preventing late complications.
