unifocalization

Surgical procedure that connects multiple abnormal blood vessels (MAPCAs) to create a single, unified pulmonary circulation.

Full Definition

Unifocalization is a complex surgical technique used in patients with pulmonary atresia and multiple major aortopulmonary collateral arteries (MAPCAs). The procedure involves disconnecting the MAPCAs from the aorta and connecting them to a central pulmonary artery or conduit, creating a unified pulmonary circulation. This allows for better control of pulmonary blood flow and may enable completion of staged palliation procedures. The goal is to establish adequate pulmonary vascular resistance and flow patterns suitable for eventual Fontan circulation or biventricular repair.

Usage

Usage note: Often hyphenated as 'uni-focalization' in some texts, but current standard is one word.

In Context

  • "The patient underwent successful unifocalization of four MAPCAs with placement of a right ventricle-to-pulmonary artery conduit." — operative note
  • "Post-unifocalization catheterization demonstrated acceptable pulmonary vascular resistance for Fontan candidacy." — catheterization report
