The Transpulmonary Gradient represents the pressure difference across the pulmonary circulation, calculated as mean pulmonary artery pressure minus pulmonary artery wedge pressure (or left atrial pressure). In pediatric critical care, TPG is used to assess pulmonary vascular resistance and distinguish between precapillary and postcapillary pulmonary hypertension. An elevated TPG (>12 mmHg) suggests increased pulmonary vascular resistance, which has implications for treatment decisions and prognosis in conditions such as congenital heart disease.
