A 19-carbon steroid hormone that serves as a precursor to both testosterone and estrone in the steroidogenic pathway. Androstenedione is produced primarily by the adrenal glands and gonads, with significant elevation occurring in conditions such as congenital adrenal hyperplasia, polycystic ovary syndrome, and androgen-producing tumors. In pediatric endocrinology, elevated levels can indicate premature adrenarche or underlying pathology causing virilization. The hormone has weak androgenic activity compared to testosterone but can contribute to clinical signs of androgen excess. Measurement is particularly useful in evaluating girls with signs of hyperandrogenism or when testosterone levels are normal despite clinical suspicion.
