Full Definition

Renal osteodystrophy encompasses the spectrum of bone abnormalities that occur in pediatric patients with chronic kidney disease, resulting from disturbances in calcium, phosphorus, vitamin D, and parathyroid hormone metabolism. In growing children, this condition can manifest as rickets, osteomalacia, osteitis fibrosa, or adynamic bone disease. Clinical features may include bone pain, skeletal deformities, growth retardation, and increased fracture risk. The condition is particularly concerning in pediatric nephrology because it occurs during critical periods of bone development and linear growth. Management involves controlling serum phosphorus, maintaining appropriate calcium levels, providing active vitamin D analogs, and sometimes using phosphate binders or calcimimetics.
