Image-Defined Risk Factors (IDRF) are specific radiologic criteria used to evaluate neuroblastoma tumors and assess the feasibility and safety of surgical resection. These factors include tumor encasement of major vessels, infiltration of adjacent organs, tumor extension across the midline, and compression of vital structures. IDRF assessment helps determine whether upfront surgery is advisable or if neoadjuvant chemotherapy should be administered to reduce tumor size and complexity before attempted resection. The presence and number of IDRFs influence treatment planning, surgical timing, and overall management strategy. IDRF evaluation requires specialized pediatric radiology expertise and multidisciplinary review.
