forced generation test

A clinical test where the patient attempts to move the eye while the examiner prevents movement, assessing muscle innervation strength.

Full Definition

The forced generation test evaluates the strength of extraocular muscle innervation by having the patient attempt to move the eye in a specific direction while the examiner restrains the eye with forceps. Unlike the forced duction test which assesses passive restriction, the forced generation test measures active muscle force generation. A positive test indicates good muscle innervation and strength, while a negative test suggests weakness or paralysis. This test is particularly useful in differentiating between restrictive and paralytic causes of strabismus, especially in cases where both mechanisms may be present, such as in thyroid eye disease or post-traumatic strabismus.
