Full Definition

Inferior oblique overaction (IOOA) is a common form of extraocular muscle imbalance in children characterized by excessive elevation of the eye when it turns inward (adduction). This condition often develops secondarily to superior oblique weakness but can also occur as a primary phenomenon. Clinical signs include a characteristic 'V' pattern in horizontal gaze measurements, with greater exotropia or less esotropia in upgaze compared to downgaze. Patients may adopt a chin-down head posture to maintain binocular vision. Treatment options include inferior oblique weakening procedures such as myectomy, recession, or anteriorization depending on the degree of overaction and presence of inferior oblique muscle restriction.
