Growth-friendly instrumentation refers to various surgical implant systems designed to control progressive spinal deformity in growing children while preserving spinal growth potential and avoiding the complications of early spinal fusion. These systems include traditional growing rods (single or dual), magnetically controlled growing rods (MCGR), vertical expandable prosthetic titanium rib (VEPTR), and shilla growth guidance systems. The devices require periodic lengthening procedures (except MCGR which lengthens externally with magnetic force) to accommodate growth until definitive fusion at skeletal maturity. Indications include progressive early-onset scoliosis, congenital spine deformities, and neuromuscular curves in growing children where observation or casting has failed. The goal is to maintain spinal alignment while allowing thoracic cage development and lung growth.
