The Pemberton osteotomy is a redirectional acetabular procedure used primarily in children with hip dysplasia who have open triradiate cartilage. Named after Dr. Pemberton, the procedure involves creating a curved osteotomy above the acetabulum from the anterior inferior iliac spine toward the triradiate cartilage, but stopping short to create a controlled 'greenstick' fracture. The acetabular fragment is then hinged downward on the intact triradiate cartilage and held with a bone graft. This technique improves anterior and lateral coverage of the femoral head while maintaining the spherical shape of the acetabulum. It is particularly useful in younger children with adequate acetabular volume.
