nephrogenic rests

Abnormally persistent foci of embryonic metanephric tissue in the postnatal kidney, recognised as precursor lesions to Wilms tumour.

Full Definition

Nephrogenic rests are microscopic clusters of metanephric blastema that persist beyond 36 weeks of gestation, when such tissue is normally expected to fully involute. They are classified as perilobar (at the periphery of renal lobes) or intralobar (within the renal parenchyma), a distinction with important implications for bilateral tumour risk and surveillance protocols. Their presence in a nephrectomy specimen must be documented, as nephroblastomatosis — diffuse or multifocal rests — substantially alters clinical management. Editors handling pathology reports must preserve this precise terminology and avoid substituting informal synonyms such as 'blastema remnants'.
