The germinal matrix is a transient embryonic structure located in the subependymal region near the lateral ventricles, composed of rapidly dividing neuronal and glial precursor cells. This region is particularly prominent between 24-32 weeks gestational age and is characterized by immature, fragile blood vessels lacking proper structural support. In pediatric neuroradiology, the germinal matrix is clinically significant as the primary site of intraventricular hemorrhage (IVH) in premature infants. The matrix normally involutes by term gestation, making hemorrhage increasingly rare in full-term babies. Cranial ultrasound is the primary imaging modality for detecting germinal matrix hemorrhages, which are graded I-IV based on extent and presence of ventricular dilation or parenchymal involvement.
