The radiocapitellar line is a crucial anatomic reference used in pediatric elbow imaging to assess for radial head dislocation. This imaginary line is drawn through the long axis of the radial neck and head and should pass through the center of the capitellum on all radiographic projections (AP, lateral, and oblique). Disruption of this relationship indicates radial head dislocation, which may be associated with Monteggia fractures, lateral condyle fractures, or congenital radioulnar synostosis. Careful assessment of the radiocapitellar line is essential in pediatric elbow trauma to avoid missing subtle dislocations that can have significant long-term consequences.
