growth failure
Impaired linear growth and weight gain commonly seen in children with chronic inflammatory arthritis and systemic rheumatic diseases.
Full Definition
Growth failure is a significant complication of pediatric rheumatic diseases, manifesting as reduced height velocity, delayed puberty, and poor weight gain. In systemic juvenile idiopathic arthritis, chronic inflammation and elevated cytokines (particularly IL-6 and TNF-α) directly suppress growth hormone action and bone formation. Local growth disturbances may also occur near inflamed joints. Corticosteroid therapy, while necessary for disease control, can further compromise growth through effects on the growth plate and growth hormone axis. Growth monitoring with serial height and weight measurements is essential, with intervention strategies including optimized disease control, nutritional support, and consideration of growth hormone therapy in selected cases.
Usage
Usage note: Distinguish from general failure to thrive; emphasize inflammatory etiology.
