JAK inhibitors are oral small-molecule drugs that selectively inhibit one or more Janus kinase enzymes (JAK1, JAK2, JAK3, TYK2), which are crucial components of cytokine signaling pathways. In pediatric rheumatology, these medications represent an emerging therapeutic option for juvenile idiopathic arthritis and other inflammatory conditions. Tofacitinib was the first JAK inhibitor approved for polyarticular course JIA in children. These drugs offer advantages including oral administration and rapid onset of action, but require monitoring for potential side effects including infection risk, laboratory abnormalities, and growth in pediatric patients.
