The Shawl sign is a characteristic rash of juvenile dermatomyositis that affects the posterior neck, shoulders, and upper back in a distribution resembling a shawl draped over the shoulders. This photodistributed rash typically presents as erythematous, scaly patches that may be mistaken for sunburn or eczema. The Shawl sign is part of the spectrum of cutaneous manifestations in JDM and may appear before or concurrent with muscle weakness.
