Tofacitinib is a selective JAK1 and JAK3 inhibitor that blocks cytokine signaling pathways involved in inflammation and immune responses. It was the first JAK inhibitor approved for treatment of polyarticular course juvenile idiopathic arthritis in patients 2 years of age and older who have had an inadequate response to one or more TNF blockers. The medication is administered orally twice daily, with dosing based on body weight. Clinical trials have demonstrated efficacy in reducing joint inflammation and improving physical function, with a safety profile similar to that seen in adults with rheumatoid arthritis.
