The Fowler-Stephens orchiopexy is a specialized two-stage surgical technique for bringing high intra-abdominal undescended testes into the scrotum. The first stage involves laparoscopic division of the testicular vessels while preserving the vas deferens and its blood supply. This forces the testis to rely on collateral circulation through the artery to the vas deferens. After 6-12 months, the second stage mobilizes the testis on its vascular pedicle for placement in the scrotum. Success rates are approximately 80-90%, with the main risk being testicular atrophy if collateral circulation is inadequate.
