Varicocele repair involves surgical correction of dilated and tortuous veins within the spermatic cord that cause scrotal swelling and may impair fertility. The condition affects 10-15% of adolescent males and can cause testicular growth retardation and decreased sperm quality. Surgical approaches include high inguinal ligation, subinguinal repair, or laparoscopic techniques. The goal is to ligate the internal spermatic vein while preserving the testicular artery and lymphatics. Microsurgical techniques reduce the risk of hydrocele formation and varicocele recurrence. Indications include testicular growth discrepancy greater than 20% or symptomatic varicoceles.
