The exstrophy-epispadias complex represents a spectrum of rare congenital malformations affecting the lower abdominal wall, bladder, and genitourinary tract. The spectrum ranges from isolated epispadias to classic bladder exstrophy to cloacal exstrophy, the most severe form. These conditions result from failure of the cloacal membrane to be reinforced by mesenchymal tissue during embryonic development. Management typically requires multidisciplinary surgical reconstruction with the goals of achieving continence, preserving renal function, and creating functional external genitalia.
