external cephalic version

Also written as: ECV — external cephalic version

Manual manipulation of the fetus through the maternal abdomen to convert breech presentation to vertex presentation.

Full Definition

A procedure performed between 36-38 weeks of gestation to manually rotate a breech-presenting fetus to vertex (head-down) position through external manipulation of the maternal abdomen. The procedure is performed under continuous fetal monitoring, often with tocolysis to relax the uterus. Success rates range from 30-80% depending on various factors including parity, amniotic fluid volume, and fetal position. Contraindications include placental abnormalities, multiple gestation, and certain fetal anomalies. The procedure may reduce the need for cesarean delivery.
