Word salad, also known as schizophasia, refers to a severe form of disorganized speech in which words and phrases are mixed together in an incoherent fashion without logical or grammatical connection. Unlike circumstantial or tangential speech, word salad completely lacks meaningful communication and may include made-up words (neologisms), rhyming, or random word associations. This symptom indicates severe thought disorder and is most commonly seen in acute psychotic episodes, particularly in schizophrenia or severe mania. Clinical documentation should provide specific examples of the disordered speech pattern to illustrate severity.
