Ultra-rapid cycling represents an extreme variant of rapid-cycling bipolar disorder, characterized by mood episodes that occur over days to weeks rather than months. Unlike standard rapid cycling (four or more episodes per year), ultra-rapid cycling involves much more frequent mood shifts, sometimes called 'ultra-ultra-rapid' when episodes occur within 24-48 hours. This pattern is associated with poor treatment response to traditional mood stabilizers, increased functional impairment, and higher suicide risk. Treatment often requires complex medication regimens and intensive monitoring, making it one of the most challenging presentations in bipolar disorder.
