Ovarian reserve is evaluated through multiple tests including anti-Müllerian hormone (AMH) levels, cycle day 3 FSH and estradiol, and transvaginal ultrasound antral follicle count. Age is the most significant factor affecting ovarian reserve, with decline accelerating after age 35. Diminished ovarian reserve may require modified treatment approaches and affects prognosis counseling.
