auto-PEEP

Unintentional positive end-expiratory pressure that occurs when insufficient time is allowed for complete exhalation before the next breath.

Full Definition

Auto-PEEP, also called intrinsic PEEP or breath stacking, develops when the expiratory time is inadequate for the lungs to return to their functional residual capacity before the next mechanical breath is delivered. This phenomenon commonly occurs in patients with obstructive lung diseases or when ventilator settings create short expiratory times. Auto-PEEP can lead to hemodynamic compromise, increased work of breathing, and ventilator dyssynchrony. Respiratory therapists must recognize and measure auto-PEEP to optimize ventilator management.

Usage note: Often hyphenated when used as a compound modifier; avoid confusing with externally applied PEEP.
