Full Definition

Plateau pressure (Pplat) is the pressure measured in the airway during a brief end-inspiratory pause when airflow is zero, representing the pressure required to maintain lung inflation. It reflects the elastic properties of the respiratory system and is unaffected by airway resistance. Plateau pressure is calculated by subtracting PEEP from total system pressure during the inspiratory hold. Normal values range from 20-30 cmH2O. Limiting plateau pressure to ≤30 cmH2O is a cornerstone of lung-protective ventilation strategies to prevent volutrauma and ventilator-induced lung injury. Elevated plateau pressures indicate reduced lung compliance or increased chest wall stiffness.
