Full Definition

Sleep onset REM episodes (SOREMPs) are episodes where REM sleep occurs within 15 minutes of sleep onset, either during nighttime sleep or daytime naps. This phenomenon is highly suggestive of narcolepsy when multiple episodes occur during a Multiple Sleep Latency Test (MSLT). SOREMPs can also occur in conditions of severe sleep deprivation, shift work disorder, or certain psychiatric conditions. The normal sleep cycle typically requires 60-90 minutes before the first REM period, making early REM onset a significant diagnostic finding in sleep medicine.
