A family of growth factors that play a crucial role in bone and cartilage formation by inducing the differentiation of mesenchymal cells into bone-forming osteoblasts. In spine surgery, recombinant human bone morphogenetic proteins (particularly rhBMP-2 and rhBMP-7) are used as bone graft substitutes or enhancers to promote spinal fusion. These proteins can significantly improve fusion rates compared to autograft alone but carry specific risks including ectopic bone formation, inflammatory reactions, and potential retrograde ejaculation in anterior lumbar procedures. FDA approval exists for specific indications with strict guidelines for use.
