Grade I represents 0-25% displacement, Grade II is 26-50%, Grade III is 51-75%, Grade IV is 76-100%, and Grade V (spondyloptosis) represents complete displacement where the vertebra has slipped entirely off the one below. This grading system is crucial for treatment planning and surgical decision-making, as higher grades typically require more extensive surgical intervention and have different prognoses.
