The highest grade of spondylolisthesis according to the Meyerding classification, representing 100% or complete anterior displacement of the upper vertebra relative to the lower vertebra. Also known as spondyloptosis, this condition most commonly occurs at the L5-S1 level and is associated with high-grade dysplastic spondylolisthesis. Grade V spondylolisthesis often presents with severe neurological symptoms, gait abnormalities, and significant functional impairment. Surgical management typically requires complex reconstruction with circumferential fusion and deformity correction.
