Exercise-induced ST elevation represents an abnormal electrocardiographic finding characterized by upward displacement of the ST segment from baseline during physical exertion. Unlike the more common ST depression seen with ischemia, ST elevation during exercise is a concerning finding that may indicate severe coronary artery disease, coronary spasm, or other serious cardiac pathology. In young athletes, this finding may also suggest conditions such as Brugada syndrome or early repolarization variants that become apparent only during exercise. The presence of exercise-induced ST elevation typically warrants immediate cessation of the exercise test and urgent cardiology evaluation. Athletes with this finding generally require comprehensive cardiac evaluation including coronary angiography before any consideration of return to competitive sports participation.
