LAST occurs when local anesthetic agents reach toxic plasma concentrations, either through inadvertent intravascular injection or absorption of excessive doses. Symptoms progress from initial CNS excitation (confusion, seizures) to cardiovascular collapse including arrhythmias and cardiac arrest. In surgical critical care, LAST may occur during regional blocks or wound infiltration. Treatment involves immediate recognition, supportive care, and lipid emulsion therapy. The condition requires rapid intervention to prevent permanent neurological damage or death.
