Lactate clearance refers to the percentage reduction in serum lactate concentration over a specified time period, typically calculated as (initial lactate - subsequent lactate)/initial lactate × 100. It serves as an important marker of tissue perfusion and metabolic recovery during resuscitation of patients in shock. A lactate clearance of ≥10% at 2 hours or ≥20% at 6 hours is associated with improved outcomes. Failure to clear lactate may indicate ongoing tissue hypoperfusion, inadequate resuscitation, or underlying metabolic dysfunction requiring further intervention.
