Blast injury refers to trauma sustained from explosive detonation and is systematically categorized into four distinct mechanisms. Primary blast injury results from the pressure wave itself affecting air-filled organs like lungs, ears, and bowel. Secondary blast injury occurs from flying debris and fragments. Tertiary blast injury results from the victim being thrown by the blast wind. Quaternary blast injury includes burns, inhalation injuries, and other trauma not covered by the first three categories. Understanding these mechanisms is crucial for trauma care as patients often have multiple, complex injuries that may not be immediately apparent, particularly primary blast lung injury which can develop hours after exposure.
