The optimal timing of SRT is controversial, with evidence supporting early intervention at lower PSA levels for maximum efficacy. Treatment planning requires careful consideration of prior surgical anatomy, risk of local versus distant recurrence, and potential toxicity to surrounding structures including the bladder and rectum.

Usage note: Distinguish from adjuvant radiation therapy, which is given immediately post-surgery regardless of PSA status.
