A retrograde pyelogram is a radiographic procedure that provides detailed imaging of the upper urinary tract by injecting contrast material directly into the ureter through a cystoscope. This technique is particularly useful when intravenous contrast studies are contraindicated due to renal failure or contrast allergies, or when better anatomical detail is required. The procedure involves placing a ureteral catheter during cystoscopy and injecting contrast in a retrograde fashion (opposite to normal urine flow). Retrograde pyelography provides excellent visualization of ureteral anatomy, strictures, stones, and collecting system abnormalities. It is both diagnostic and therapeutic, as it allows for simultaneous interventions such as stent placement or stone manipulation.
