The aortic neck is the relatively normal-caliber segment of infrarenal aorta located between the lowest renal artery and the beginning of aneurysmal dilatation. In endovascular aneurysm repair (EVAR), the aortic neck serves as the proximal sealing zone where the endograft must achieve secure fixation and seal against the aortic wall. Neck characteristics including length, diameter, angulation, thrombus burden, and calcification are critical determinants of EVAR feasibility and long-term success. Inadequate neck anatomy is a primary reason for EVAR exclusion and may necessitate open surgical repair or fenestrated/branched endografts.
