Radioguided occult lesion localization (ROLL) is a surgical localization technique for non-palpable breast lesions that involves placing a radioactive tracer (typically technetium-99m) into or adjacent to the target lesion under imaging guidance. During surgery, a gamma probe detects the radioactive signal to guide precise excision of the lesion while minimizing removal of normal breast tissue. ROLL offers advantages over wire localization including improved patient comfort, scheduling flexibility, and potentially more accurate lesion removal. The technique can be combined with sentinel lymph node biopsy and may result in better cosmetic outcomes due to more targeted tissue removal.
