Coronary flow velocity represents the speed of blood movement through coronary vessels and can be measured invasively using Doppler-tipped guidewires or non-invasively through transthoracic echocardiography. The measurement provides important hemodynamic information beyond angiographic appearance, including assessment of microvascular function, coronary flow reserve, and the physiological significance of stenoses. Normal coronary flow velocity patterns show characteristic systolic and diastolic phases that vary between the left and right coronary circulations. Abnormal flow velocity patterns may indicate epicardial stenosis, microvascular dysfunction, or competitive flow in bypass grafts. The technique is particularly useful for evaluating intermediate lesions and assessing the success of interventional procedures at the microcirculatory level.
